The direction of rotation is determined by an occlusal view of the tooth  The appropriate Wedge indent into the aligner is created with a gentle squeeze of the instrument on the labial, near the line angle of the tooth, adjacent to the facial embrasure.  Then the same instrument is positioned on the lingual surface at the opposite line angle to create the rotational couple.
